Question I need to find a form

A friend of my Dad's has adopted us, and we her, because she has no family around and we help one another. She is going into the hospital to have a repair done for a prolapsed bladder and the hernia in the vagina it is trying to poke through. I and my Dad will be watching her 9yr old twins while she is in the hospital for a week and I need a form to print out for each of them so I can have the authorization to seek medical attention for them if needed. Steven and Amber have Ehlers-Danlos Syndrome where the ligaments are loose and the blood vessels are fragile; they also are double-jointed due to this.

Could you ask their pediatricians or the hospital where she is going to be admitted ? I would think that you need a proper legal form for this. If you cant get to the doctors office beforehand, maybe they could email it to you and you could print it out that way.

My mom always wrote a note and kept it with us giving consent with her signatures. But here is a site that may help you with a form:

Medical Permission

I think the former poster who said that the hospitals and doctors' offices might have the forms you need is right. That's what I did when taking care of a neighbor's pets this summer. Also, we notified the vet that I was responsible for the pets for x amount of time. I would get your forms notarized to be extra sure. It is so smart of you to plan ahead. What good friends you are.

It is only right and she has nobody else to trust and she is scared if DHS ever gets the twins . . . I enjoy those two 'house apes' and they both adore Sean so we'll get along . . . I've been made 'aunt' :p and being an 'only' it feels good

When I sent my kids to spend the summer with their grandmother all I did was print a Limited Power of Attorney (off the internet)and specified what it was for. I had it notorized and she never had a problem using it.

Make your own There is no form police!

Just sign and date it with everybody's name on it and the kids brithdates and pediatrician info. I clip their medical card to it. I make one form and add all the friends names and addresses and phones numbers who watch my kids. The form goes in my kid's bag with the kid. I do make a point of showing it to the adults once so they know it is there.

Thank you all for the advice; I'll get a form printed up for each child . . .and every tag agency has a notary public so I will have no problem getting it notarized.

Hi! I am an ER registrar, in plain language that means I take the insurance when you come into the ER. All you need is a notarized letter stating that you are able to make medical decisions for the children. Some addt'l info you might want to have: allergies,up to date on shots?,pediatrician,surgeries?,any chronic medical condition?, medications,insurance,the primary insurance carriers ss#,dob,employer and here is the kicker.... religion. People get very touchy w/that question. They get annoyed at me as if I am asking for my own personal knowledge.

Some of it is pretty straight forward. She is on SSI and the kids have state insurance.
