I tried the trick to test my dd's thermos for hot foods, and it didn't keep the hot water very hot. I guess it's not very well insulated. I have an insulated insert that came with a baking dish carrier - one is for hot & the other is for cold. The hot one, you just microwave for a few minutes and it keeps the food hot for hours. I'm going to check around for miniature sized ones.
DD's been happy with pb & j sandwiches a couple time a week and she requested pepperoni & cheese slices. They have snack so close to lunch time, that she hasn't really been hungry enough to eat all her lunch. I always pack a sweet treat (like a mini-candy bar or rice crispie treat), but unless they eat everything else in their lunch, they can't have their dessert.
I'm going to try Sue's idea of putting bite sized foods on a toothpick.
