You know your child better than anyone. My only suggestion would be to remain cautious. I, too, quizzed my child about her feelings when these accidents happen. Your daughter may be responding to the emotional angst she must feel when they occur, as well as the family's distress over the issue.
It could be solely emotional, but the experts tell me that the overwhelming majority of cases come from physical sources (the emotional stuff happens as a result).
