I know. They have been in this area for a few years. A local hospital gave them out. I filled out one for my FIL.....although I actually didn't use their form. He had too much info/medications to fit  so I typed it all out on Word Perfect using their format and then put it in the vial after printing it out! I update it when any changes are made.
Luckily, his has never been needed. But better safe than sorry!
He also carries a copy of the info in his wallet!
