Is the dog spayed?
I had a cat years ago that did that.....problem was the she wasn't spayed... just a thought.
You might keep in mind that some animals have traits/ habits that you aren't going to be able to change. I had a shelter cat that would potty in the litterbox, step out, and pee on my floor. Every time. I had to take her back to the shelter. They worked with her for a month, and figured she will always do that, and was unadoptable. I felt bad, but I couldn't allow her to tear up my house, either.
